Urgent notifications should be telephoned to the Yorkshire and Humber HPT within 24 hours on 0113 386 0300 – please refer to Department of Health Protection Legislation (England) Guidance 2010 (http://www.dh.gov.uk/)

Routine notifications for Yorkshire and Humber should be completed via the online form Notification (NOID) Form for any of the following:
