Rightcare Barnsley service

The RightCare Barnsley Integrated Single Point of Access (SPA) acts as a single point of access for referrals into the six Integrated Neighbourhood Teams which includes both administrative and clinical triage to ensure that people get the support they need, when they need it, to prevent them escalating to crisis point.

The service provides a telephonic single point of access for health and social care staff with its primary function of providing care coordination and hospital admission brokerage regarding the correct level of care required.

The primary aims of the SPA are to receive and distribute patient referrals for SWYPFT physical health community services via telephone / email using TPP SystmOne and to prevent unnecessary emergency hospital admissions, reduce length of stay in hospital whilst awaiting discharge support, minimise delayed transfers of care (DTOC) and reduce emergency hospital re-admissions. 

Some elements of the response service are time critical (can be 999 emergency response or minimum 2 hour for receiving service) public expectation and support of other services.

 

Accessing the service

The service currently operates from 08:00 to 20:00, seven days per week, 365 days per year.
