Safeguarding Adult Review

Date: 12 November 2018

Safeguarding Adults Review:

Following the very sad death in a house fire early this year of “Jack”, a 68 year old Barnsley gentleman, the Barnsley Safeguarding Adults Board commissioned a Safeguarding Adults Review (SAR). A SAR is held when an adult dies as a result of abuse or neglect whether known or suspected and there is a concern that partner agencies could have worked more effectively to protect the adult or when an adult in the area has not died, but the SAB knows or suspects that the adult has experienced significant abuse or neglect. The aim of a SAR is to promote learning and improvement action in order to prevent future incidents involving death or serious harm.

This short briefing paper has been written by the Barnsley Safeguarding Adults Board. It gives an overview of what happened and outlines the key learning points for professionals.

The full SAR report can be found at: https://www.barnsley.gov.uk/media/9273/safeguarding-adult-review-jack.pdf
