  • Infertility in Women-Investigations and Referral Guidance***

    This referral form seems to contain ALL the clinical content and investigations required for fertility problems especially for the female partner prior to Fertility Service referral. Also contains menstrual diary, anatomy picture, patient info sheet, National guidelines on fertility problems and the  Yorkshire and Humber Fertility policy .

  • Menopause and HRT Hormone Replacement Therapy guidelines***

    Women's and Sexual Health

    Contains basic menopause and HRT guidelines. It  also contains various useful patient resources- information leaflets on alternatives to HRT, food fact sheets during the menopause, useful websites and NICE Guidelines on menopause and HRT. Finally the FSRH guidance on contraception after the age of 40 years .

    This includes the guidance for GPs in terms of Hx , examination, investigations and appropriate referral.

    I have also included a diagnostic flow chart regarding how the various diagnosis are made. This may be more secondary care related , however, I thought it was important to include here, both for interest and explanation to the patients as what to expect in secondary care .
