BREATHE

BREATHE are a multidisciplinary team providing specialist treatment and support, seven days per week, for patients with a diagnosed respiratory condition. The service offers patient centred care in clinics around Barnsley to make it easily accessible and, if required, will support patients in their own home to manage their condition.

Once you have been referred to the BREATHE service you will be contacted and invited to a clinic appointment or a home visit will be arranged if that is more appropriate. The BREATHE staff member will begin to assess your needs and talk a little more about what the service can offer. Also, they might suggest onward referral to support services such as pulmonary rehabilitation.

  • Early supported discharge and post discharge support from secondary care
  • Oxygen assessment and long term management
  • Ambulatory oxygen assessment
  • Long term nebuliser management
  • Complex case management

Accessing the service

BREATHE will accept referrals from healthcare professionals for patients that meet the inclusion criteria and can offer the following service:

  • Early supported discharge and post discharge support (including provision of short-term nebulisers if required)
  • Home oxygen assessment for stable patients N.B patients requiring oxygen assessment for hospital discharge should be referred to the inpatient oxygen team
  • Ambulatory oxygen assessment
  • Long term nebuliser assessment and review
  • Complex case management

Referral guidelines

Exclusion criteria:

  • Heart failure (unless requiring community oxygen assessment)
  • Acute chest infections
  • Pulmonary Rehabilitation
  • Sleep disordered breathing

Patients with the following respiratory conditions will be seen by the team:

  • COPD
  • Bronchiectasis
  • Cancer – EOL
  • Pneumonia
  • LRTI with diagnosed respiratory condition or requiring specialist respiratory input
  • Respiratory rehabilitation pathways (including COVID management and rehabilitation)
  • Other chronic respiratory conditions
