Dietetics - Community Nutrition and Dietetic Service
The Community Nutrition and Dietetic Service provides specialist dietetic advice for patients requiring nutrition support as well as those with a range of conditions including Diabetes, IBS, coeliac disease and other gastrointestinal disorders. The service also provides specialist dietetic intervention for adults requiring home enteral nutrition support.
Services provided
The service provides care at clinic locations throughout Barnsley and offers home visits to care home residents and those who are housebound.
A dietitian will gather anthropometric measurements and review blood results, to carry out a full dietary assessment enabling them to provide individualised, evidence based dietary advice.
Referrals for adults and children are accepted into the service. Written and verbal advice is provided with each assessment.
Training can also be provided to service users, their carers and other health professionals regarding a wide range of dietary issues.
Support offered:
- One-to-one appointments offered as telephone or face to face
- Home visits for people who are housebound
- Group sessions
- Provision of written information to support advice given
Accessing the service
Base: The Cudworth Centre, Carlton Street, Cudworth, Barnsley, S72 8SU.
Hours: Monday to Friday, 8.30a am-4.30 pm
Who should be referred
Referral Criteria
Inclusion criteria for adults:

- Nutrition Support (underweight, undernourished, unintentional weight loss).
- Diabetes (Please note we do not accept direct referrals for Diabetes. Referrals must be made via the Barnsley Hospital Diabetes SPA).
- Gastro-related conditions (e.g. IBS / IBD / Coeliac Disease).
- Enteral Tube Feeding (e.g. PEG, NG, NJ)
- Dietetic Assessment (including symptom management)
Exclusion criteria for adults:

- Individuals requiring weight management (Obesity) support (please refer to Tier 3 Weight Management)
- Individuals with eating disorders:
- Individuals with a recent stroke diagnosis with a stroke-related nutritional problem (please refer to Barnsley Community Stroke Team)
- Individuals with a learning disability diagnosis who have a nutritional problem (please refer to Barnsley.LD.Duty@swyt.nhs.uk)
- Individuals who are currently under the care of a Paediatrician, please use the Paediatric referral form for this cohort
Inclusion criteria for children:

- Faltering Growth
- Gastro-related conditions (e.g. IBS / coeliac disease / constipation)
- Cow’s Milk Protein Allergy. (Please confirm diagnosis before referring for dietetic support)
- Food Allergy / Food Intolerance (other than cow’s milk protein allergy)
- Selective Eating (please note we do not provide feeding therapy)
- Dietetic Assessment (including symptom management)
Exclusion criteria for children:

- Children requiring weight management support (please refer to Tier 3 Weight Management)
- Children with eating disorders. (Please refer to BarnsleyCYPMHRequestSupport@swyt.nhs.uk)
- Children with suspected or confirmed IgE allergy / complex or multiple food allergy. (Please refer to Barnsley Hospital Paediatric Allergy Clinic)
- Children with diabetes
- Children with an enteral feeding tube.
