Tier 3 Weight Management Scheme
Barnsley’s tier 3 weight management service helps people to achieve a healthier weight and improve their overall wellbeing. The service utilises diet and lifestyle interventions such as improving the overall quality of a diet, increasing physical activity levels, and implementing behavioural strategies. In addition, the service has support from medical and psychological staff for those wishing to pursue pharmacological treatments, receive psychological input and/or bariatric surgery.
The service is delivered across multiple formats dependent upon which pathway you choose to follow. These formats include either being seen face-to-face within a clinic, receiving telephone consultations, or even accessing our online videos and virtual group sessions.
The service works closely with other supporting services including GP practices and specialist hospital teams.

- For suitable clients, the service can make onward referrals to the tier 4 service for bariatric surgery assessments
- Staff have received specialist training in nutrition and motivational interviewing to ensure a high-quality service is delivered
- The service has specialist medical support from adult and paediatric consultants based at Barnsley hospital
- The service works closely with the local tier 2 weight management service, GP practices and specialist secondary care teams
Support offered
The service delivers the following:
- Weight loss support for obese adults across the borough. This may include some/all of the following; conservative weight loss measures, the use of pharmacological interventions and assessments/preparation for patients wishing to have bariatric surgery
- Antenatal weight management input for obese ladies
- Children between the age of 4-15 years old with a BMI above the 98thcentile
- Access to specialist multi-disciplinary team (MDT) clinics for adults and children

Referral guidelines
Referrals also accepted from:
GPs, nursing staff, health visitors, school nurses, midwives, AHPs and consultants. Please note that we do not accept self-referrals from patients.

Who should be referred
Referral criteria:
Adults:
- ≥16yrs
- BMI >35kg/m2 with 2 or more co-morbidities i.e. Type 2 diabetes, Hypertension etc
- BMI > 40 kg/m2 without co-morbidities
- Client meeting the national referral criteria for bariatric surgery and age 18 and above:
- BMI >30kg/m2 with type 2 diabetes requesting bariatric surgery
- BMI >40kg/m2 without co-morbidities
- No specific uncontrolled metabolic or psychological reason for obesity
- Specialist antenatal weight management (For pregnant women with a BMI over 30kg/m2)
Please note that domiciliary visits can only be done in extenuating circumstances e.g. housebound, as this does not form part of the service specification.
Paediatrics:
- Between 4 – 15yrs
- BMI adjusted for height and gender above the 98thcentile
- BMI adjusted for height and gender above the 91st centile with significant comorbidities and complex needs for example learning disabilities and or other additional support needed for instance safeguarding issued directly related to obesity
- Does not have a diagnosis of diabetes or an active eating disorder
Exclusion criteria:
Services not specifically stated as part of this specification are excluded. Any services provided outside the terms of the agreement shall have the CCG’s specific consent in advance.
Excluded are:
- Clients who do not satisfy the referral criteria
- Clients with unstable or severe mental health problems
- Physical illness beyond the expertise of primary care
- Clients with severe active eating disorders
