Barnsley Universal Health Passport: Information for health and social care staff 

Local people said they wanted a simpler way to communicate with health services, helping them to get the right care and support in the ways that work for them. 

To support people attending appointments, we’ve worked with health services, children, young people and adults across Barnsley to co-produce our new Universal Health Passport. 

This passport has been designed to help professionals across health and social care understand people’s communication preferences, care and support needs, and any reasonable adjustments they may need. 

Being completed and owned by people themselves, we hope this simple tool will support you in delivering more personalised care and treatment.
