NHS Low Calorie Diet Pilot (Commissioned nationally by NHS England) – Went Live 1st September 2020
- The Low Calorie Diet Pilot across SYB aims to have 500 participants across the first two years of the pilot take part across all five areas on a one to one coach to patient delivery model. Recognising the small number across the five areas PCN Clinical Directors and Coordinators were contacted at all 37 networks/neighbourhoods to have a discussion about the launch and how we plan to manage referrals. We are one of ten nationally commissioned sites selected to deliver the pilot.
- The pilot is 12 months in duration for a patient to go through – in recognition of this it would be ideal for those more motivated patients identified by Clinicians to be referred opportunistically rather than by clinical system search as quality of referral and patient are key to evaluate the outcomes of the pilot
- There are 3 phases first 12 weeks is a total diet replacement, 13 to 18 weeks is the food reintroduction stage 19 to 52 is regular and ongoing maintenance and support from the Coach
- Sessions will commence using Microsoft Teams with patients receiving their TDR supplements, glucose, BPM kits through the post which are sent by the Provider who are REED Wellbeing
- Referral forms are available on Systm 1 and EMIS in addition to the referral any medication adjustments a patient may need to support them on their journey (as referred to in the attachment) must be provided in written form and a copy given to the patient and a copy sent to the Provider.
- As the pilot is a one to one delivery it is envisaged when safe to do so REED will see patients in a practice setting face to face which is why we are keen to reiterate to PCN’s to identify a host practice or more than one in some areas where patients could be seen when safe to do so – physical face to face sessions aren’t expected until currently spring time and are subject to national guidance.
