Interim External Evaluation of the Specialist Care Home ANP role - Sees Improved Care for Residents

An interim external evaluation of the Specialist Care Home ANP role  project has been completed. This focused on the outcomes for residents of care homes in Barnsley’s South and Central neighbourhoods between 1 March 2019 and 29 February 2020.

It found that with Katie’s input:

  • 95% of the residents supported died in their preferred place of care
  • 88% of the residents had an emergency care plan created to support decision making in and out of hours
  • the specialist knowledge that Katie brings was shown to increase the confidence of care home staff when caring for residents with palliative care needs and provide reassurance and comfort residents and their families.

This specialist practitioner post is funded by Macmillan Cancer Support and hosted by South West Yorkshire Partnership NHS Foundation Trust as a 3-year project. The post is occupied by former Macmillan Nurse and end of life practice educator Katie Yockney. Working closely with local care homes, neighbourhood teams, GPs and hospital colleagues, she offers help to develop personalised care plans for care home residents. These plans help to support care home staff and other health care professionals to meet a person’s preferences and wishes at the end of their lives.
