Food Allergy Reporting - commercial premises referral form

If the anaphylactic or allergic reaction occurred either at or after visiting a commercial premise

(such as restaurant, take-away or fast food outlet) we need to notify The Local Authority.

This is to enable them to visit the premises and ensure that food allergy standards are being met
