Neighbourhood Nursing and Community Matron Service
The Neighbourhood Nursing Service is essential in preventing unnecessary admission to hospital.
Neighbourhood Nurses (District Nurses) use their skills, knowledge, and experience to assess when home-based treatment is the best option for nursing a particular patient. The team offers treatments including complex wound care, assessment and management, catheter care, blood sampling for house bound patients, first infusion Ivs in community, palliative care, health education, and promotion. The service is provided 8.00 am – 8.00 pm, 7 days per week. The out of hours element to this service is provided by our Urgent Community Response service (01226 644575).
Urgent Community Response (UCR) provides 2hr crisis response for patients, as outlined in the UCR clinical guidance documentation NHS England » Urgent community response services. The service works 24/7, 365 days per year. Referrals for UCR can be made via ringing RightCare Barnsley Single Point of Access on 01226 644575.
Proactive Care Community Matrons provide assessment and support for individuals, within their own home or place of residence, who have complex physical health needs requiring co-ordinated care, who are severely frail (Rockwood frailty score of 7 or above) and are at risk of hospital admission or escalation. They may also have had frequent unplanned hospital admissions / emergency department attendances and require further support to prevent readmission. The matron team provide support Monday – Friday 9am – 5pm
Hospital at Home / Virtual Ward Community Matrons support two-hour and urgent response requirements for adults with complex physical health conditions who require advanced clinical assessment within crisis or urgent timescales. This includes patients on acute-level virtual ward pathways who require face-to-face Advanced Clinical Practitioner assessment, patients at risk of hospital admission or clinical escalation, and patients referred via Yorkshire Ambulance Service escalation routes where urgent clinical decision-making may prevent conveyance or further deterioration. They are supported by the wider Urgent Community Response and virtual ward teams. The Hospital at Home / Virtual Ward Community Matron provide support 7 days per week 8am – 6pm.
